Our forms are designed for use with a variety of scanners. Many are fully compatible with popular Optical Mark Read scanners, such as Scantron™ and NCS Pearson™. We also design test forms for image scanners using Teleform™ software by Cardiff. This technology allows our clients to digitally capture handwritten answers and other open-response data.
Custom Form Design -- Step by Step
1. Your account representative will perform a needs analysis by asking you questions like the ones below:
- What type of test-scoring machine will scan the form?
- What is the maximum number of questions the tests will have?
- What response pattern is desired?
- What identification fields will be required?
- Do you want your form to be pre-slugged with respondent identifiers?
- Will any branding/logos be required on the form?
2. Our composition department will lay out the answer sheet and prepare a proof, usually within three to seven days, depending on the complexity of the form design. Once approved, the artwork will be submitted to our production facility for printing.
